Levels of Support Coordination

Level 1 - Support Connection (0106)

Support Connection helps you understand your NDIS plan and connect with the right providers, community services and mainstream supports. It focuses on building your confidence and skills so you can manage your supports more independently over time.

Level 2 - Coordination of Supports (0106)

Coordination of Supports helps you put the right services in place to achieve your goals and navigate your NDIS plan with confidence. Your Support Coordinator works alongside you to coordinate providers, strengthen your support network and build your capacity to make informed decisions and live more independently.

Level 3 - Specialist Support Coordination (0132)

Specialist Support Coordination is for participants with more complex support needs who require a higher level of assistance. A Specialist Support Coordinator helps manage challenging situations, coordinates multiple services and works to ensure your supports are effective, consistent and responsive to your individual circumstances.

Finding the Right Providers - Choosing providers shouldn't be confusing. We work with a broad network of trusted organisations and independent providers, helping you find services that match your goals, preferences, culture and location.

Coordinating Your Supports - Managing appointments, communication and multiple providers can become overwhelming. We work behind the scenes to ensure everyone involved in your support network is working together toward the same outcomes.

Building Your Independence - Support Coordination is about creating long-term independence. We will help you develop the knowledge and confidence to make decisions, advocate for yourself and increasingly manage your own supports if that aligns with your goals.

Preparing for NDIS Reviews - As your goals change, your supports should evolve too.

We help gather reports, monitor outcomes and prepare for NDIS plan reassessments to ensure your funding reflects your current needs.
